Send the Standard Written Order
Fax a completed SWO to 509-980-7062, email it to rx@petersonmedicalequipment.com through a secure channel, or call 509-783-7501 and we'll walk through it with your office.
We check it & handle the paperwork
We confirm the SWO carries all six required elements, verify the face-to-face documentation is within range, and — for the codes that need it — open the prior authorization and request the written order prior to delivery.
Fulfillment & records
We dispense the device, keep your office informed, and archive the signed order and supporting documentation — indexed by NPI and date, retained for ten years.

What a Standard Written Order has to include
Per CMS Article A55426, a valid SWO carries six elements. Miss one and the order isn't complete:
- The beneficiary's name or Medicare Beneficiary Identifier (MBI)
- The order date
- A general description of the item — HCPCS code, brand/model, or a narrative description
- Quantity, if applicable
- The treating practitioner's name or NPI
- The treating practitioner's signature
Separately, applicable items require a face-to-face encounter documented within the six months before the order (42 CFR 410.38), and certain spinal orthoses require a Written Order Prior to Delivery and prior authorization — see the spinal orthoses page.

A verified web portal, with roster upload
We're building a portal to replace the fax-back loop entirely. Here's the shape of it:
Verified accounts
Prescriber accounts are checked against the NPPES registry and the OIG exclusion list at signup, with multi-factor sign-in and short idle timeouts. Office staff can prepare orders; only the credentialed physician can sign.
Roster upload
Upload a patient roster as a spreadsheet and the portal builds a draft order for each row — pre-checked for the obvious problems (state, face-to-face window, code validity). The physician then signs each order individually, as Medicare requires; the portal just removes the busywork around it.
Guided signing
A signing flow walks the physician through every valid draft, re-authenticating periodically, and produces a signed PDF with a certificate of completion for each order.
Automatic archive
On signature, the order is delivered to our orders queue, written to the records archive at a predictable path, and tracked to fulfillment — with a tamper-evident audit log behind it.
